GOOD GUIDE FOR PARENTS – NEW GEN PARENTING

 

Joint families, earlier were the nurseries for molding behavior and provided the thumb rules to co-existence for children. But with families becoming nuclear and smaller’ bringing up kids has become a tough task

There are no words of wisdom from grandparents or uncle and aunts and children have to grow up in a world dominated by television and computers. The time to look after kids is also in short supply if both the parents are working; the quality of time that children get is also poor or adulterated.

In such a world, Parenting New Gen is both an eye-opener and a guide. The author is both a doctor and a mother of two. Hence, she combines theory along with practical wisdom in her book.

The first few chapters deal with Planned Parenthood and introduction to parenting the remaining deal with personality development of children from infancy to late adolescence. The chapters on temper tantrums, discipline, punishment and TV viewing have a practical approach. They deal with behavioral aspects, especially as the child deals with objects of the world. The chapter-Comprehensive care of the child, deals with the physical aspects and includes information on immunization, hygiene and spiritual well-being. The chapters on “Parents with Problems” takes a look at handicapped children and behavioral disorders including lying, stealing and sibling rivalry. This book also has an immunization schedule.

TRAIN UP A CHILD – NEW GEN PARENTING

It is a curious fact that a job which entails the highest degree of responsibility should require no sort of training at all. The well-being of future generations depends upon this onerous task; yet it requires no qualifications… Strange though it may seem, just about anybody can be a parent!

The damage both to individuals and to society caused by poor parenting can never really be estimated, it is true that volumes have been written about parenting since time immemorial. Yet the need for a set of guidelines still exists, Dr. Grace Lalana’s New Gen Parenting offers practical advice especially to young parents and those on the threshold of parenthood. The first chapter deals with planned parenthood and the second with an introduction to parenting.

Coming down to brass tacks, the subsequent chapters gives a clear picture of personality development in children – the king of information essential to child rearing. However the section that follows Comprehensive Child Care has been written from a wealth of experience and specialized knowledge and is easily the back bone of the work. Some of the problems faced by parents today have been skilfully addressed in the next section. The final chapter advocates a positive mental attitude and gives useful hints on how to achieve it.

One of the most important factors to be kept in mind by all parents and educators, namely discipline has been effectively and quite exhaustively dealt with in the book. Discipline means different things as the child goes through different stages of development-a point the author is at pains to make, The parent of teach (or guardian) must be sensitive to the constantly changing and growing physical, psychological and spiritual needs of the child, right from age 0 to adulthood. Valuable information on nutrition and immunization has been given in tabular form, easy for reference.

This book is to be recommended highly for use by those in search of reliable material on the topic of parenting.

GROWTH PATTERNS IN CHILDREN WITH CONGENITAL HEART DISEASES

 

I am extremely privileged to have been asked to write a foreword for this outstanding treatise written by Dr. Grace J. Lalana. I will be the first one to admit my limitations as a non pediatric cardiologist to comment on issues of growth patterns in children with congenital heart disease. However it has been a learning experience for me to examine and thus learn much of the very detailed work contained here. A large number of diagnostic tools are referred to here that deals with the assessment of growth in children with congenital heart disease. The ultimate goal has been to understand and estimate the degree of malnutrition and thereby offer corrective measure for the benefit of this group of cardiac infants and children, I like to commend Dr. Lalana for this excellent work. I hope this book will be a source of information for those involved in the care of these infants and children. RECENT ADVANCES IN PERINATOLOGY-WITH SPECIAL REFERENCE TO ETHNICITY

“Recent Advances in Perinatology – with special reference to ethnicity,” authored by Dr. Grace J. Lalana, based on some hard core research done by her on ethnic mothers and newborns. It is commendable that she is promoting the concept of ‘Perinatology’ and team work through her book. I am glad that, gone are the days when it was “I (obstetrician) am in charge until the cord is cut and you (Pediatrician) take over from that point.” We have to have the two principal specialists liaising from the early antenatal days and in some cases at the stage of the mother planning a pregnancy. This is quite logical as conception, fetus and newborn are all progressive stages of a continuous process of differentiation, maturation, growth and development. This coordination will lead to better outcome. We should encourage ‘regionalization’ of newborn care and in utero transfers. Perinatal centers should be established which will have services of all the constituents of Perinatal practice like Obstetrics, Neonatology, Pediatric surgery, ultrasonography, genetic and metabolic laboratories, fetal medicine, counseling services, ‘high risk mother and baby’ clinics and developmental assessment centre.

Dr Lalana has given us new insight into fetal maturation with respect to Asian babies which has significant implication on planning of delivery and management of the newborn. She has dealt with the important aspects of Perinatal Medicine like the ‘fetal medicine’’,’perinatal care’,’at risk pregnancy’, ‘congenital malformations’ ‘fetal assessment ‘and ‘management of the newborn.’The highlight of the book is the data on Perinatal epidemiology. Chapters on ‘Perinatal trends’, ‘Classification of newborns’, ’risk score colour index’ and ethnic specific nomenclature and definitions are very interesting. These topics are seldom discussed in Obstetric or Neonatalogy textbooks. She has also provided (see appendix) the Asian, Caucasian and Eurasian due dates chart. I am sure that this book will be very useful to Obstetricians, Pediatricians and all those who are involved in the care of mother and newborn.

Her recent ground breaking research discovering for the first time in the world an effective non-invasive newly born resuscitation which physiologically initiates the first all-important breath by sustained nasal oxygen inflation (SNOI) also alleviates hypoxia so that children are normal.

RISK SCORE COLOR CODING INDEX

Assessment of newborn by gestation age is outdated; neonates should be assessed by risk score-color coding index which takes into consideration not only gestational age but also birth weight and the influence ethnicity plays in the outcome of the neonate. The early, quick identification of high risk Asian and Caucasian newborn is facilitated by using the Risk-score and Color-Coding index, which is also a convenient method for coding and classifying all newborns based on birth weight, gestation and intrauterine growth pattern gives quick identification of those newborns needing special to intensive care. However complications at birth need to be dealt with immediately.

A new ethnic specific due dates for delivery for Asian and Caucasian population has also been designed taking into consideration the earlier maturation and birth of ethnic Asian newborns as compared to their Caucasian counterparts thus outdating the old common expected date of delivery at 280 days or 40 completed weeks of gestation and ushering a new Asian due date for delivery at 39 weeks gestation and new Caucasian due date for delivery at 41 weeks gestation catering for the best outlook of the newborn. These definitions have been arrived at, after in-depth analysis of various aspects of 21,585 consecutive in- births during the five year

period 1979 to 1983 in Child Health unit II and Neonatal Services, at the Christian Medical College and Hospital, Vellore, South India.
